What is the Trauma Timeline worksheet?

The trauma timeline worksheet pdf gives clients a two-page grid for recording significant traumatic events in chronological order. Each row pairs an Event and Date with the client’s Emotional Reaction and Physical Reaction. Ten numbered rows let a client build the sequence gradually rather than in one sitting.

How to use the Trauma Timeline worksheet

Use this worksheet both in session and as homework. In session, start with the Event and Date columns only, so the client builds a factual sequence before adding affect. Once the sequence is stable, move across to Emotional Reaction and Physical Reaction one row at a time. Your clients can pace themselves by completing two or three rows between appointments and recording anything unexpected in the Additional Notes box. Stop the exercise if arousal rises faster than the client can settle.

Trauma Timeline Worksheet Download – What’s Included

The download is a two-page printable PDF. Page one carries rows one to five, page two carries rows six to ten, so a long history does not get compressed into a single sheet. Every row has four columns: Event, Date, Emotional Reaction and Physical Reaction. Page two closes with an Additional Notes area for observations that do not belong to any single event.
