The Cognitive Triangle CBT worksheet is a one-page, illustrated handout, not a fill-in form. A diagram links Situation, Thoughts, Emotions and Behaviors, and each corner carries a definition and examples. A six-step application walks one presentation at work from the first thought to a balanced replacement.

How to use the Cognitive Triangle CBT worksheet

Use this sheet both in session and as homework. Draw the client’s own example onto the Situation corner before showing the printed one, which assumes an office presentation. Move around the triangle in the order the sheet gives, Thoughts then Emotions then Behaviors, because clients often report the feeling first and skip the thought. Your clients can do steps five and six alone once the first four are familiar.
